It is to the great and lasting credit of LORENZ BOHLER and his school that they have in the last decade developed and demonstrated so thoroughly the techniques for the conservative management of fractures. Nevertheless there have always been many, including some from BOHLER'S school, who have found considerable place for surgical management, and with the significant progress in general surgery seen in postwar years, a new stimulus has been given to this part of traumatic surgery, especially since bone injuries have become more complex and frequent. The concept of internal fixation is not new. The serious criticisms that have been levelled at it retain today their basic significance. Progress in the fields of asepsis, corrosion-free metal implants, operative experience and postoperative care has diminished the dangers but has not relieved the surgeon of responsibility. The Association for the Study of the Problems of Internal Fixation (AO) has devoted itself over a number of years to the basic principles and best methods of open treatment of fractures by means of extended clinical and scientific studies in order to determine in each individual case the most promising line of treatment. At the same time a well designed and tested instrument set has been developed with precise instructions for the appropriate techniques. As a result, the new observations about primary bone healing which have emerged from the practice of rigid internal fixation are as interesting as the uses to which they can be put in allowing early mobilization.

The German edition of our book entitled "Operative Frakturen behandlung" by M. E. MUELLER, M. ALLGOEWER and H. WILLENEGGER (Springer, Berlin . Goettingen . Heidelberg, 1963) has been out of print now for more than three years. We are planning a new edition which will deal with the collective experience of 14,000 new cases, all treated by internal fixation, and will include the newest developments in the field of internal fixa tion. However, it will be some time before this new edition can be published. Increasing demands for a description of the AO technique of internal fixation has stimulated us to publish this manual. In it we shall describe in a comprehensive but somewhat apodictic mann er the principles and techniques of the AO methods of fracture treatment and reconstructive surgery, which in our hands, have stood the test of time. The book is written in a somewhat abbreviated style. It cor responds in subject matter to the teaching given at the AO courses in Davos, but deals with each subject more thoroughly. We have dispensed with pictures of the instruments, as these may be found in the Synthes Catalogue *. This manual should be regarded as the product of collective experiencc, containing new thoughts and new discoveries from basic research. In considering the risks of mistakes and dangers, we can only reiterate what we have already stated in "Technique of Internal Fixation of Fractures": "Open treatment of fractures is a valuable but difficult method which involves much responsibility.

The danger of infection remains the most serious drawback to internal fixation. Prevention, using all available prophylactic measures should be the central feature in every surgical department. Though the infection rate may remain below the acceptable level of 2 %, the infected patient derives little comfort from the large number of excellent results in other people. We must discover the best way to offer these patients a favourable prognosis. This should not be based on intuition but on clear proven guiding principles. Planning the treatment for a patient whose internal fixation has become septic must decide between steps which may have advantages or disadvan tages. The disadvantage of the presence of a foreign body must be weighed against the advantage of rigidity. The authors have taken up this challenge by planned animal experiments to study the healing of internally fixed fractures which have been infected with staphylococci. They have shown that under stable conditions, even massive infection did not destroy the healing process in cortical bone. Even under these circumstances fracture union, in the form of primary bone healing, can occur even if with less regularity than in unin fected internal fixation.

More than a century ago, Cripps successfully employed the direct and complete division of the anal sphincters as a means of approaching the lower rectum and anal canal, and reported on a series of 36 patients who had been treated in that fashion. Cripps was pleasantly surprised to find good fecal continence in over two-thirds of the patients during later follow-ups, despite the fact that the divided sphincters had not been repaired. The transsphincteric procedure was largely forgotten in subsequent years, however, and only the parasacral proctotomy of Kraske, which spared the anal sphincters, can be said to have gained an established place in the" surgical armamentarium. " It remained for York Mason to redirect the attention of the surgical community to the great potential of the trans sphincteric approach and the excellent continence that can be achieved through adequate repair of the divided sphincters. Having recognized the outstanding practical value of this procedure, we felt it necessary to define more precisely the anatomical prerequisites that would ensure minimum operative bleeding, and to bring the procedure more in line with current knowledge of normal continence and defecation. Dr. A. Huber, in consultation with the director of the Institute for Clinical Anatomy of our surgical department, Prof. A. von Hochstetter, did many months of dissection work on fresh anatomic preparations in an effort to explore and refine the various aspects of the trans sphincteric ap proach."

It is with pleasure that we offer these introductory remarks for the Manual of Internal Fixation in the Horse, a book describing a further application of AO or ASIF techniques. The letters A-O stand for the Arbeitsgemeinschaft fUr Osteosynthesefragen and have been trans lated into the Association for the Study of Internal Fixation. The organization is truly a "study group," created in Switzerland, that met for the first time in 1958. The major goal was to establish a task of fracture treatment by force committed to the improvement osteosynthesis. The group's motivation arose out of the then prevailing unsound or inconsistently successful attempts at fracture treatment. According to statistics obtained from the Swiss National Health Insurance Program at the time, the so-called conservative treatment of fractures had resulted in a high rate of persistent morbidity. The problems encoun tered included: irreparable damage due to long-term immobilization; delayed union or pseudoarthrosis; malalignment; and, inadequate reduction of intraarticular fractures with resultant osteoarthritis. Accurate, stable osteosynthesis seemed the only practical way to address those various shortcomings. However, many of the osteosyn theses performed at that time had led to new problems, since most were not stable and, in some cases, actually worked to prevent healing."

The AOjASIF* dynamic hip screw (DRS) has been designed primarily to stabilize trochanteric fractures of the hip. Selected fractures of the femoral neck and some subtrochanteric fractures are further indications for the DRS [40, 46]. The dynam ic condylar screw (DCS) has been developed for fractures of the distal femur and is now being tested clinically. The DRS and DCS are carefully coordinated with the preexisting ASIF standard sets of equipment for internal fixation of fractures. The concept of a sliding screw for trochanteric fractures is not new. The first author describing such an implant was Schumpelick [44]; he gives credit to Pohl [22], who was primarily a manufacturer working for Gerhardt Kiinscher. Re described the possibility of impaction at the fracture site with a sliding device. In the United States Clawson [7, 8] introduced the hip screw and found it to be extremely beneficial in trochanteric fractures. At approximately the same time, Massie [31, 32] and Pugh [39] designed the sliding-type flange nails, which offer similar intramedullary splinting with the possibility of fracture impaction. The following chapters describe the concept and design features of the DRS, as well as the details of the surgical technique. The application of the DRS for different types of fractures is illustrated with clinical examples. The results of 268 cases of trochanteric fractures treated with the DRS are presented and compared with results using the angled blade plate and Ender's nails. Finally, some laboratory tests are described.

Der Zugang zum tiefen Rektum und zum Analkanal durch direkte Spaltung des gesamten Sphinkterapparates wurde vor uber 100 Jahren durch Cripps erfolgreich angewendet und von ihm eine entsprechende Serie von 36 Patienten veroeffent- licht. Cripps war dabei angenehm uberrascht, trotz fehlender 2 Rekonstruktion bei uber j der Patienten anlasslich von Spat- 3 kontrollen gute Kontinenz zu finden. Die Moeglichkeit geriet aber in der Zwischenzeit weitgehend in Vergessenheit, und im "chirurgischen Armamentarium" hat sich eigentlich nur die parasakrale Rektotornie nach Kraske gehalten, welche den Sphinkterapparat moeglichst unbehelligt lasst. Es ist das Verdienst von Y ork Mason, die mannigfachen Moeglichkeiten des sphinkterspaltenden Zugangs erneut und uberzeugend dargelegt und auf die ausgezeichnete Kontinenz- funktion bei adaquater Rekonstruktion hingewiesen zu ha- ben. Nachdem wir uns von der Nutzlichkeit des sehr pragma- tisch durchgefuhrten Eingriffs uberzeugt hatten, schien es uns unbedingt erforderlich, die anatomischen Voraussetzungen eines moeglichst blutsparenden Vorgehens zu prazisieren und dieses Vorgehen in Beziehung zu setzen zu den heutigen Kenntnissen der normalen Kontinenz respektive Defaka- tionsfunktion. Herr Huber hat sich - beraten durch den Vorsteher des Instituts fur klinische Anatomie im hiesigen chirurgischen De- partement, Herrn Prof. A. von Rochstetter- anhand frischer Dissektionspraparate uber ein Jahr mit den Fragen dieses Zugangs befasst. An den klinischen Fallen dieser Zeit konnten dann seine Erkenntnisse immer wieder in ihrer Bedeutung fur die chirurgische Praxis uberpruft werden. Die entspre- chenden gemeinsamen Erfahrungen sind in der kleinen Mo- nographie niedergelegt.

Seit der 1. Auflage gab es so wesentliche Neuerungen, vor allem in der Therapie, dass es notwendig war, das Werk fast voellig neu zu konzipieren und zu schreiben. Wie ausserordentlich das Wissen in der Gastroenterologie in den letzten Jahren angewachsen ist, zeigt quantitativ der Umfang des Buches. Eigentlich gibt es jetzt keine 2. Auflage, sondern eine annahernd komplette Neukonzeption. Der Begriff "Chirurgische Gastroenterologie" beinhaltet sowohl die gastroenterologische Chirurgie als auch das notwendige Wissen, um eine adaquate Diagnostik und Entscheidungsfindung bei gastroenterologischen Erkrankungen herbeizufuhren. Das Buch vermittelt nicht nur das moderne Wissen zur Pathophysiologie und Pathogenese gastroenterologischer Erkrankungen, sondern auch die zielstrebige Diagnostik und Indikationsstellung zur gastroenterologischen Chirurgie. Der interessierte Leser kann alle notwendigen Informationen finden, die zum Verstandnis gastroenterologischer Erkrankungen und chirurgischer Entscheidungen notwendig sind - beschrieben von den jeweiligen Kapazitaten. Damit ist das Buch nicht nur fur Chirurgen wertvoll, sondern auch fur Gastroenterologen und Internisten, die sich uber die chirurgischen Moeglichkeiten bei Erkrankungen des Verdauungstrakts informieren moechten. Auch Vertreter anderer an der Behandlung gastroenterologischer Erkrankungen beteiligten Disziplinen wie Roentgenologie, Strahlentherapie, Nuklearmedizin, Onkologie etc. finden wesentliche Informationen.

Operationen an den abfuhrenden Harnwegen, insbesondere am Pyelon und Ureter, sind haufig geworden. Um so wichtiger ist die Kenntnis der Pathophysiologie des Harnabflusses zur Beurteilung der Indikation zur Operation und der Operationserfolge. Dies gilt nicht nur fur die verschiedenen Ersatzplastiken, sondern auch fur die transplantierte Niere, die in dieser Richtung noch kaum untersucht ist. Hier eine systematische Grundlage fur vergleich- bare Untersuchungen geschaffen zu haben, ist das Verdienst des vorliegenden Beitrages von Herrn RUTISHAUSER. Obwohl der Allgemeinchirurg diese Funktionsdiagnostik gerne dem Facharzt uberlaBt, bleibt er doch an der grundsatzlichen Problematik interessiert und ist fur gute Orientierung dankbar. Das vorliegende Werk erfiillt in glucklicher Weise zwei Aufgaben, namlich einer- seits diejenige der genauen Instruktion des Facharztes und andererseits eine zusammenfas- sen de Darstellung der Grundprinzipien fur den Allgemeinchirurgen. Dem Buch darf ich guten Erfolg und dem Autor aufbauende Kritik und Anregungen wunschen. Fruhjahr 1970 M. ALL GOWER VORWORT Die urologische Diagnostik verfiigt zur Beurteilug der oberen Harnwege iiber eine Reihe bewahrter Methoden der Rontgenkontrastdarstellung. Mit ihrer Hilfe lassen sich Erkrankungen, die mit morphologischen Veranderungen am Hohlraum- system einhergehen, zuverlassig erkennen. Fiir die Erfassung funktioneller Symp- tome sind diese "klassischen" Verfahren indessen weniger geeignet. Mit Ausnahme der Rontgenkinematographie, deren Anwendung aber ebenfalls Grenzen gesetzt sind, konnen sie zur ErschlieBung der immer noch ungeniigend geklarten Physiologie und Pathophysiologie von Pyelon und Ureter kaum mehr als Hinweise beitragen. Mehr AufschluB ist von der kontinuierlichen manometrischen Beobachtung der Harnwegsperistaltik zu erwarten.

Die sturmische industrielle Entwicklung, die zunehmende Verarbeitung von energiereichem, hoch explosivem Material zu Friedens- und Kriegszwecken und die Ansammlung von Menschen, die mit gefahrlichen labilen Stoffen auf relativ kleinem, meist auch geschlossenem Raum hantieren mussen, lassen die Zahl der Verbrennungsunfalle zwangsma13ig anwachsen. Nach H. A. BAXTER! sterben daran schatzungsweise in jedem Jahr sechzigtausend Menschen. Dazu kommen Hunderttausende, die, mit verschiedensten Verbrennungsschaden mehr oder weniger lang hospitalisiert, fUr den Arbeitsproze13 ausfallen. Uber die Not- wendigkeit neuer prophylaktischer Ma13nahmen und ihre volkswirtschaftliche Bedeutung schreibt BAXTER: "When one calculates the staggering number of man-hours of work lost to industry, the hospital beds required, and the hours of medical and nursing care needed, it is indeed remarkable that so few preventive measures have been adopted. The widespread use of noninflammable or fire- resistant clothing by the civilian population would greatly reduce the number of cases requiring treatment on this continent each year. " Die biologisch-medizinischen Aspekte del' thermischen Einwirkungen und ihre! Folgen sind seit etwa 100 Jahren die gleichen geblieben. Den lokalen wundchirur- gischen und plastisch-operativen Problemen stehen die weniger durchsichtigen Fragen del' Allgemeinreaktion des Organismus gegenuber. Stets wird die Relation zwischen Ausdehnung del' Verbrennung und del' Prognose diskutiert. Fur den Fruhtod nach ausgedehnter Verbrennung werden toxische Stoffe im Blut (BILL- ROTH, zitiert nach SONNENBURG 2), Lahmung des Tonus del' Hautgefa13e und Leer- 3 laufen des Herzens (FALK ), reflektorische Herabsetzung des Gefa13tonus mit letalem Kollaps durch nervose Irritation (SONNENBURG!) verantwortlich gemacht.
